Flyers with entry forms are at most Oklahoma City area motorcycle shops.  Fill out and sign one of these forms or print the one below. Mail it to Street Enduro LLC or take it to Performance Cycle 6200 NW Expressway and ask for Richard Meadors or Ajax Kawasaki 8417 S. I-35 and see Amy at the parts counter.  Both of these locations have a limited number of event packets.  Call David Fogle at 405 620-2726 if you need additional entry information.
